New York Codes, Rules and Regulations, Title 10, Section 2.59 (10 NYCRR Section 2.59). 10 NYCRR Section 2.59 requires any Article 28, 36, or 40 healthcare facility or agency to document the number and percentage of personnel vaccinated against influenza for the current season and report these data to the New York State Department of Health.

This information is only collected once a week from hospitals. The title of this dataset was initially the Hospital Electronic Response Data System (HERDS) Hospital Survey: Hospital Staff COVID-19 Vaccinations. The dataset was changed to its current title on 11/4/2021.

The contract laboratory provides the radon test results to the individual home owner and the Radon Program. All testing data is entered into our database. From this database, we are able to create radon prevalence maps, design special outreach activities and campaigns, and track the location in the home where the detector was placed.

This dataset is a subset of data which only includes basement testing with average pCi/L greater than 4.

Effective July 14, 2009, physician offices that perform surgical or invasive procedures using more than mild sedation must be accredited by one of three accrediting agencies. The NYSDOH collects OBS practice site accreditation data from each of the accrediting agencies monthly.
