The dataset groups 65 individual Potentially Preventable Complication (PPC) measures into 8 different categories, providing observed and risk-adjusted rates for all payer discharges by hospital and statewide, beginning in 2013.
Potentially Preventable Complications (PPC), obtained from software created by 3M Health Information Systems, are defined as harmful events or negative outcomes that develop or occur during hospitalization and may result from processes of care and treatment rather than from natural progression of the underlying illness.
The PPCs were calculated using Statewide Planning and Research Cooperative System (SPARCS) inpatient data.

This dataset provides the results from collecting and testing adult deer ticks, also known as blacklegged ticks, or by their scientific name Ixodes scapularis. Collection and testing take place across New York State (excluding New York City) from October to December, when adult deer ticks are most commonly seen.
Adult deer ticks are tested in “pools”, or groups of up to ten adult ticks per pool, for the Powassan virus, also known as Deer tick virus. These data should simply be used to educate people that there is a risk of coming in contact with ticks and tick-borne diseases.
These data only provide adult tick minimum infection rates at a precise location and at a point in time. Both measures, tick population density and minimum infection percentages, can vary greatly within a very small area and within a county. These data should not be used to broadly predict disease risk for a county.

Admitted means that the patient was newly admitted to the hospital or was confirmed positive after admission. Gender information became available for COVID-19 admissions as of May 2, 2020. Hospitalized means patients admitted as inpatients in either inpatient or observation beds and does not include patients that were treated and released from an Emergency Department. The title of this dataset was initially the Hospital Electronic Response Data System (HERDS) Hospital Survey: COVID-19 Admissions by Gender. The dataset was changed to its current title on 11/4/2021.

Sepsis is defined as a clinical syndrome in which patients have an infection that is accompanied by an extreme systemic response. Sepsis of sufficient severity that the function of major organ systems in the body (such as heart, kidney, brain, and others) is impaired is referred to as “severe sepsis.” Patients with severe sepsis that have continued organ system impairment and/or low blood pressure that does not respond to treatment with adequate fluid replacement are considered to be in “septic shock.” The combination of early detection of sepsis coupled with timely, appropriate interventions can significantly improve the chances of survival for patients with all types of sepsis.
The datasets contain hospital-level treatment measures and hospital-level risk-adjusted mortality rates for adult patients, with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health in 2015 and 2016.
The treatment measures and risk adjusted mortality rates are presented by hospital. Only hospital level measure data for which there were at least ten cases in the denominator is reported. Statewide measures are calculated using all hospitals, regardless of the number of sepsis cases treated.

This dataset includes Medicaid Managed Care, Commercial HMO, and Commercial PPO performance data from the Quality Assurance Reporting Requirements (QARR) by member demographic characteristics. QARR is largely based on measures of quality developed and published by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS®). Plans are required to submit quality performance data each year. Demographic information analyzed in this report includes members’ sex, age, race/ethnicity, Medicaid aid category, cash assistance status, behavioral health conditions including serious mental illness (SMI) and substance use disorder (SUD), payer status, and region of residence. Measuring the quality of care, and the ability to measure disparities in care is an important first step to a better understanding of the underlying factors that drive differences in care among certain populations within Medicaid Managed Care, Commercial HMO, and Commercial PPO.
*Due to COVID-19 impacts, measures requiring hybrid review are not included in this dataset for year 2019.

This dataset includes information on school reported COVID-19 testing and case positive data beginning on September 13, 2021. Data is collected from charter schools on each operational day using the daily school survey form, which school administrators access by logging in to the NYSDOH school survey website. Each school may submit data for the current operational day between the hours of 7am and 5pm. At 5pm, a process runs which aggregates data at the school and district levels. The following morning the data are updated for the previous day and displayed on the School COVID Report Card website.

This dataset represents hospital-reported data for all adult patients with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health. Quality measures are calculated using this data for reporting hospital-specific rates and trends. The Adult New York State sepsis process of care measures were developed using a National Quality Forum (NQF) measure for guidance: NQF #500 Severe Sepsis and Septic Shock: Management Bundle. These measures, reported as statewide and hospital-specific rates, reflect several key processes of care that can increase the probability of surviving an episode of sepsis. There is one important outcome measure, mortality, included in this dataset for which statewide mortality rates and hospital-specific risk adjusted mortality rates are reported.
