This data is collected real-time and updated monthly. The dataset consists of subject's: Name, Address, License Number, License Type, Year of Birth, Effective Date, Action, Misconduct Description, and the Date Updated. For an individual physician or physician assistant search, the most efficient way to obtain requested information is through the search feature on the DOH Program Homepage.
