- Disclaimer
- Data is reported by health plans. Certain fields (including the Medicaid Provider Identification Number) are plan-specific identifiers that are used to link to the health plan’s own data. This field may contain numbers, symbols, or characters, and cannot be used to identify a specific site across health plan networks. The NYSoH consolidated the way health plans submitted individual networks starting with Q3 2018. Instead of having each plan submit for each separate product they were offering, health plans began submitting different networks under separate network indicator fields.

- Provider/facility site locations are submitted by multiple health plans and some may report the same data in different ways. Address information is standardized, but practice names are not.
